Our Lash Extension Service Waiver outlines the terms and conditions associated with our eyelash extension services. This waiver is designed to ensure you are fully informed about the process, understand the potential risks, and agree to our service protocols. Please read through this waiver carefully to ensure a clear understanding of the procedures and policies before proceeding with your lash extension service at Lash Lab Express.

Procedure Consent and Liability Waiver
Procedure Consent:
I authorize a qualified independent contractor at Lash Lab Express, referred to as my lash technician, to perform semi-permanent eyelash extension procedures. I understand this involves attaching synthetic eyelashes to my natural lashes with adhesive. It is imperative that I remain still and keep my eyes closed throughout the procedure.
Risks and Complications:
I acknowledge that my lash technician has thoroughly explained the eyelash extension process. I understand there are risks, including but not limited to temporary eyelash loss, eye redness, irritation, or allergic reactions to products used.
Informed Consent and Procedure Discomfort:
I consent to the procedure, fully aware of the risks. Should discomfort occur, I will promptly inform my lash technician, who will make every effort to address the issue, including stopping the session if necessary.
Health Disclosure:
I have disclosed all relevant health information and medication history to Lash Lab Express and my lash technician. I understand that unforeseen conditions may arise during or after the procedure.
Post-Application Care:
I understand the importance of aftercare for maintaining my eyelash extensions. I will avoid excessive moisture for the first 24 hours and adhere to the recommended care instructions. Failure to follow these guidelines may result in complications.
Release of Liability:
I release and hold harmless Lash Lab Express and my lash technician from any liabilities, claims, or damages related to the eyelash extension procedure, even if due to negligence on their part. This release is comprehensive and extends to any claims by my family, heirs, or personal representatives.
No Refund Policy:
I understand that Lash Lab Express does not offer refunds. However, service adjustments are available within three days of the initial appointment. If an allergic reaction occurs, resulting in visible puffiness or discharge, Lash Lab Express will remove the extensions free of charge within three days.
Dispute Resolution:
Any disputes will be resolved through binding arbitration with the American Arbitration Association. The arbitration will occur in the city and state where my lash technician is based. I will bear all associated legal costs in enforcing this agreement.
